Code Collaborator provides a Process Customization for the Save Work Item, and Save Change Set Links and Comments Operation Behaviors

Configuring both will do three things:

1. When a Work Item state changes a review is created, linked changes are uploaded and approvers are added to the review
2. When a a Change Set is linked to a Work Item with a review, it is uploaded to that review
3. When new approvers are added they are to the review

Further, you can configure what states and work item types trigger review creation.
